Background:

  • Radius-Maumenee syndrome (RMS) is a rare idiopathic condition.
  • Characterized by enlarged episcleral vessels and elevated intraocular pressure (IOP), leading to secondary glaucoma.

Observation:

  • A 23-year-old male presented with chronic eye redness and headaches.
  • Examination revealed episcleral venous engorgement, elevated IOP (28 mmHg RE), optic nerve damage, and visual field defects.

Findings:

  • Diagnosis of RMS was confirmed after excluding other orbital pathologies.
  • The patient exhibited secondary open-angle glaucoma consistent with RMS.

Implications:

  • This case underscores the association between enlarged episcleral vessels and glaucoma.
  • Ophthalmologists should consider RMS in cases of unexplained glaucoma with episcleral vascular changes.
