Background:

The GRBAS scale is a standard for subjective voice quality assessment.
Objective voice analysis tools are needed to complement subjective evaluations.
GRBASzero is a novel tool designed for automated voice analysis.

Purpose of the Study:

To evaluate the clinical reliability and validity of the GRBASzero tool.
To determine if GRBASzero meets requirements for clinical voice testing.

Main Methods:

Assessed GRBASzero reliability and validity using two independent datasets (n=283 and n=287).
Compared GRBASzero ratings with human expert evaluations (GRBAS).
Employed Fleiss Kappa for reliability and intraclass correlation coefficient for validity.

Main Results:

GRBASzero demonstrated poor test-retest reliability, with features A and S showing random consistency.
Agreement between GRBASzero and human experts was poor across all features except B in dataset 1.
Dataset 2 also revealed poor agreement between GRBASzero and human expert ratings.

Conclusions:

GRBASzero exhibits inadequate reliability and validity for clinical use.
The tool requires substantial optimization before it can be reliably applied in clinical settings.
Current performance indicates GRBASzero does not meet the necessary standards for clinical voice assessment.
