Background:

A 2022 report emphasized limited transparency in organ offer declines and urged patient involvement in organ offer decisions.
Guidance on integrating patients into organ offer decisions is currently lacking.
This study investigates waitlist patients' experiences and perceptions of a new Organ Offer Review Card (OORC).

Purpose of the Study:

To explore kidney waitlist patients' preferences for decision-making in organ offers.
To assess patient perceptions of a novel Organ Offer Review Card (OORC) prototype.

Main Methods:

A prototype Organ Offer Review Card (OORC) was developed using Donornet refusal codes.
Sixty kidney waitlist patients at a single center were invited to a web-based survey.
The survey focused on current medical decision-making preferences and OORC perceptions.

Main Results:

Of 43 patients reached, 17 completed the survey (39.5% response rate).
Most participants (88.2%) deemed involvement in organ offer decisions important; 100% desired to know reasons for organ declines.
The OORC prototype was perceived to enhance understanding of organ selection factors (94.1%) and increase trust in the transplant team (88.2%).

Conclusions:

An Organ Offer Review Card (OORC) can significantly improve transparency and communication in the organ transplant waitlist process.
Implementing an OORC may foster greater trust between transplant teams and patients.
Patient engagement tools like the OORC are crucial for equitable organ transplantation.
