Summary

Benign paroxysmal positional vertigo (BPPV) is predicted by stroke, hyperlipidemia, chronic suppurative otitis media, cervical spondylosis, and osteoporosis. A developed nomogram accurately assesses BPPV risk.

Background:

Purpose of the Study:

  • To identify independent risk factors for BPPV.
  • To develop and validate a predictive nomogram for BPPV risk.

Main Methods:

  • Retrospective analysis of 312 participants (164 BPPV patients, 148 controls).
  • Univariate and multivariate logistic regression analyses to identify risk predictors.
  • Construction and validation of a clinical predictive nomogram using concordance index and calibration plots.

Main Results:

  • Stroke, hyperlipidemia, chronic suppurative otitis media, cervical spondylosis, and osteoporosis were identified as independent risk factors for BPPV.
  • A nomogram was constructed with a regression equation: logit(P) = -6.820 + 0.450*stroke + 0.312*hyperlipidemia + 0.499*CSOM + 0.916*cervical spondylosis + 0.628*osteoporosis.
  • The nomogram demonstrated excellent predictive accuracy and clinical applicability for risk thresholds between 20% and 60%.

Conclusions:

  • Stroke, hyperlipidemia, chronic suppurative otitis media, cervical spondylosis, and osteoporosis are significant independent predictors of BPPV.
  • The developed nomogram provides a valuable tool for predicting BPPV risk in clinical practice.
