    Background:

    • Elevated intraocular pressure (IOP) is a primary risk factor for glaucoma.
    • Traditional glaucoma therapies may have significant complications.
    • Combined surgical approaches are being explored for complex cases.

    Purpose of the Study:

    • To evaluate the efficacy of combined keratoplasty and trabeculectomy in patients with high IOP.
    • To assess the safety and outcomes of this combined surgical procedure.

    Main Methods:

    • Seven patients with a mean preoperative IOP of 40 mm Hg underwent combined keratoplasty and trabeculectomy.
    • Postoperative IOP was monitored throughout the follow-up period (mean 16.1 months).

    • The average postoperative IOP significantly decreased to 23.7 mm Hg.
    • Five out of seven eyes achieved well-controlled IOP during the follow-up period.

    Conclusions:

    • Combined keratoplasty and trabeculectomy can be a successful treatment for lowering high IOP in glaucoma patients.
    • This surgical strategy may offer a viable alternative, potentially mitigating risks seen in other glaucoma interventions.
