    Intentional tooth ankylosis can help correct severe maxillary retrusion by using ankylosed deciduous teeth for extraoral traction, enabling maxillary movement without unwanted tooth displacement.

    Background:

    • Maxillary retrusion treatment is often limited by tooth movement when using conventional extraoral force.
    • Conventional orthodontic treatment using maxillary teeth for extraoral force can lead to undesirable tooth movement and periodontal remodeling.

    Observation:

    • A novel technique utilizing intentionally ankylosed deciduous teeth as abutments for delivering extraoral force was applied in a patient with severe maxillary retrusion.
    • Anteriorly directed intermittent extraoral force was applied to the ankylosed teeth for 12 months, successfully correcting an anterior crossbite.

    Findings:

    • Cephalometric analysis confirmed that the occlusal correction resulted from anterior maxillary movement.
    • The ankylosed teeth remained stable, showing no significant movement, while the maxilla advanced.
