    Summary

    A postoperative carotid bruit in asymptomatic patients does not always indicate significant residual disease. Duplex scanning helps evaluate the carotid artery noninvasively after endarterectomy.

    Area of Science:

    Background:

    • Carotid endarterectomy is performed to prevent stroke.
    • Postoperative bruits can suggest persistent or recurrent carotid artery lesions.

    Observation:

    • 18 asymptomatic patients with postoperative bruits underwent Duplex scanning.
    • 23 carotid endarterectomies were evaluated 6 weeks to 2 years post-surgery.
    • Indications included transient ischemic attacks, stroke recovery, and asymptomatic stenosis.

    Findings:

    • Duplex scans showed normal or mild/moderate thickening in most vessels.
    • Doppler velocities and velocity ratios (VIC/VCC) were within a wide range.
    • The presence of a bruit did not correlate with hemodynamically significant lesions.

    Implications:

    • Postoperative bruits after carotid endarterectomy may not require immediate intervention.
    • Noninvasive Duplex scanning is valuable for assessing carotid artery status post-surgery.
    • Further investigation may be needed for asymptomatic postoperative bruits.
