Summary

This study found no causal link between glaucoma and stroke using Mendelian randomization. Glaucoma management does not appear to influence stroke risk, and stroke diagnosis is not a predictor for glaucoma.

Background:

Purpose of the Study:

  • To investigate the potential causal relationship between glaucoma and stroke.
  • To determine if glaucoma influences the risk of developing different types of stroke.

Main Methods:

  • Utilized two-sample Mendelian randomization (MR) with large-scale genome-wide association studies.
  • Analyzed data from over 20,000 glaucoma cases and 260,000 controls, and 80,000 stroke cases and 630,000 controls.
  • Employed inverse-variance weighted (IVW), weighted median, and MR-Egger methods for robust analysis and sensitivity testing, adjusting for metabolic factors.

Main Results:

  • No causal role was found for glaucoma or its subtypes (primary open-angle, primary angle-closure) in relation to any stroke, ischemic stroke, large-artery atherosclerotic stroke, small-vessel stroke, or cardioembolic stroke.
  • These null associations were consistent across European and Asian populations and remained robust after adjusting for metabolic traits.
  • Reverse MR analyses also showed no significant causal effect of stroke types on glaucoma risk.

Conclusions:

  • Large-scale Mendelian randomization analyses do not support a causal relationship between glaucoma and stroke.
  • The association between glaucoma management and stroke risk prevention requires careful evaluation in future research.
  • Stroke diagnosis should not be used for glaucoma risk prediction.
