Ovarian Cycle01:27

Ovarian Cycle

The menstrual cycle includes a critical component known as the ovarian cycle, which undergoes two main phases each month—the follicular phase and the luteal phase. The follicular phase is variable and averaging around 14 days. Ovulation, triggered by a surge in luteinizing hormone (LH), marks the transition between the two phases. The second phase, the luteal phase, is relatively consistent, lasting approximately 14 days, and is marked by the activity of the corpus luteum. While a cycle length...

Area of Science:

  • Obstetrics and Gynecology
  • Maternal-Fetal Medicine

Background:

  • Uterine incarceration is a rare obstetric complication.
  • Commonly presents with maternal pain and urinary retention.

Observation:

  • A case of uterine incarceration during the first pregnancy required surgical reduction under general anesthesia at 27 weeks.
  • This was complicated by preterm delivery at 29 weeks.
  • Recurrent symptoms in the second pregnancy led to earlier diagnosis and manual reduction at 19 weeks.

Findings:

  • Earlier diagnosis and manual reduction of uterine incarceration in a subsequent pregnancy were successful.
  • Locoregional anesthesia was used for the second reduction.
  • The patient achieved term delivery in the second pregnancy.

Implications:

  • Recurrent uterine incarceration can be managed with earlier, less invasive interventions.
  • Timely diagnosis and digital maneuver can lead to successful term delivery.
  • This case highlights the importance of recognizing recurrent symptoms for improved maternal and fetal outcomes.
