Background:

Electronic health record (EHR) use is integral to modern surgical practice.
Understanding gender-based differences in EHR utilization is critical for addressing physician well-being and potential disparities.

Purpose of the Study:

To investigate and quantify gender differences in EHR usage patterns among attending surgeons.
To identify specific areas where time allocation and documentation practices differ between male and female surgeons.

Main Methods:

A cross-sectional study analyzed EHR data from 224 attending surgeons at an academic hospital.
Data included patient encounters, time spent in the EHR, note length, and documentation volume.
Statistical analyses, including Kruskal-Wallis tests and multivariate linear regression, were employed to compare outcomes between genders.

Main Results:

Female surgeons spent more time per note and wrote longer inpatient and outpatient notes compared to male surgeons.
While overall time in the EHR was similar, female surgeons spent more time logged in outside of standard clinic hours.
Male surgeons completed more medical records per month, but female surgeons manually wrote a higher fraction of their notes.

Conclusions:

Female surgeons face a disproportionately higher documentation burden within the EHR system.
These findings highlight potential factors contributing to burnout and inform strategies for promoting gender equity in surgical professions.
Addressing these EHR-related workload differences is crucial for physician well-being and fair compensation.
