Observation:

  • A 65-year-old male presented with a firm, dark-brown, non-mobile conjunctival mass.
  • Initial clinical suspicion was conjunctival malignant melanoma (CMM).
  • Ocular and systemic examinations were otherwise normal.

Findings:

  • Histopathological examination revealed dilated vascular channels consistent with cavernous hemangioma.
  • No evidence of malignancy was found, refuting the initial diagnosis of CMM.
  • The lesion was characterized by vascular channels, red blood cells, and fibrous interstitium.

Implications:

  • Conjunctival cavernous hemangioma can present with clinical features mimicking malignant conjunctival lesions.
  • This benign vascular tumor may be misdiagnosed as CMM or other aggressive conditions.
  • Accurate histopathological diagnosis is essential to avoid unnecessary aggressive treatment for benign conditions.
