Aortic valve (AV) repair without aortic root replacement has a significant mid-term reoperation rate for aortic regurgitation (AR) recurrence. Patients with more than mild AR post-discharge require careful monitoring for potential reoperation.
Area of Science:
- Cardiovascular Surgery
- Cardiac Valve Repair
- Aortic Valve Disease
Background:
- Aortic valve (AV) repair is complex, with a steep learning curve and variable reproducibility.
- Durability and validity of AV repair without aortic root replacement require further investigation.
Purpose of the Study:
- To examine mid-term outcomes of AV repair without aortic root replacement.
- To identify factors associated with reoperation due to aortic regurgitation (AR) recurrence.
Main Methods:
- Retrospective analysis of 14 patients undergoing AV repair without aortic root replacement (March 2007 - May 2018).
- Comparison of baseline characteristics and postoperative outcomes between patients requiring reoperation (Group R) and those not (Group F).
- Statistical comparison of AR recurrence rates and associated factors.
Main Results:
- Median follow-up was 5.5 years; 35.7% of patients required reoperation for AR recurrence.
- No significant differences in baseline characteristics between groups.
- Patients requiring reoperation had significantly higher AR grade before discharge (p = 0.013).
Conclusions:
- The indication for AV repair without aortic root replacement may need reassessment due to a considerable mid-term reoperation rate.
- AV repair cases with more than mild AR at discharge warrant close monitoring for future reoperation.
