Background:

  • Cardiothoracic surgical trainees report a need for enhanced instruction in robotic-assisted thoracoscopic surgery.

Purpose of the Study:

  • To assess the current utilization and trainee exposure to robotic surgery within US thoracic surgery residency programs.
  • To identify factors influencing the adoption of robotic surgery by graduating trainees.

Main Methods:

  • A voluntary 10-question electronic survey was distributed to surgeons in US thoracic surgery residency programs.
  • Data collected included residency size, robot availability/usage, and post-graduation robotic surgery adoption.
  • Multivariable logistic regression analysis was performed to identify significant predictors.

Main Results:

  • Surveys from 69 of 76 (90.8%) programs were analyzed. Robotic surgery was used for 55% of pulmonary lobectomies.
  • 35 of 69 programs (50.7%) have a formal robotic curriculum.
  • Robotic surgery adoption post-graduation was significantly higher for thoracic track trainees (97.5%) compared to cardiothoracic (56.4%) and cardiac (10.1%) tracks. A formal curriculum (OR, 5.82) and larger program size (OR, 3.78) were associated with higher adoption.

Conclusions:

  • Significant disparities exist in robotic surgical training across different cardiothoracic residency programs.
  • Standardized curricula and formal case requirements are likely necessary to ensure optimal preparation of future graduates in robotic surgery.
