Summary

Primary spinal cord gliomas have poorly defined features and high mortality. Immunohistochemistry markers like H3K27me3 loss and p53 positivity predict poor outcomes in these rare tumors.

Background:

  • Primary spinal cord gliomas are rare tumors with high mortality and poorly understood clinicopathological features.
  • Unlike brain tumors, specific diagnostic criteria and prognostic markers for spinal cord gliomas are not well-established.

Purpose of the Study:

  • To analyze the clinicopathological, histopathological, and immunohistochemical features of primary spinal cord gliomas.
  • To identify prognostic factors influencing overall survival (OS) in patients with spinal cord gliomas.

Main Methods:

  • Retrospective analysis of clinical data, histopathology, and immunohistochemistry (IHC) for IDH1 R132H, H3K27M, p53, and H3K27me3 in 25 patients.
  • Correlation of IHC findings and histopathological grades with overall survival (OS).

Main Results:

  • Gliomas were graded as WHO 1 (16%), 2 (20%), 3 (8%), and 4 (52%). One case had a rare molecular profile (H3K27-altered).
  • H3K27M positivity was found in 36% of patients, and H3K27me3 loss in 52%.
  • H3K27me3 loss, high p53 positivity (≥10%), high MIB-1 index (≥10%), and high histopathological grades significantly correlated with poor OS.

Conclusions:

  • Immunohistochemistry markers, including H3K27me3 loss and p53 expression, are crucial for assessing prognosis in primary spinal cord gliomas.
  • These pathological and immunohistochemical characteristics provide valuable insights into the prognosis of spinal cord gliomas.
