Summary

Endovascular aneurysm repair (EVAR) for abdominal aortic aneurysm (AAA) can lead to a significant decrease in hemoglobin (Hb) levels and the development of anemia, particularly in patients with hypertension. Pre-operative Hb levels may offer protection against post-EVAR anemia.

Background:

  • Low hemoglobin (Hb) levels are linked to adverse outcomes after abdominal aortic aneurysm (AAA) surgery.
  • The relationship between AAA volume changes and Hb alterations following endovascular aneurysm repair (EVAR) is not well understood.

Purpose of the Study:

  • To investigate changes in Hb levels and AAA volume before and after EVAR.
  • To determine the association between post-operative Hb levels and AAA volume loss.
  • To identify risk factors for anemia developing 24 hours after EVAR.

Main Methods:

  • Retrospective analysis of 74 patients undergoing EVAR for AAA between January 2020 and February 2021.
  • Calculation of AAA volume from CT angiography images pre- and post-EVAR.
  • Analysis of Hb levels, clinical features, and medical history to identify anemia risk factors.

Main Results:

  • Hb levels significantly decreased 24 hours post-EVAR, with post-operative Hb negatively correlated with AAA volume loss (r = -0.252, p = 0.041).
  • Anemia developed in 59.5% of patients within 24 hours post-EVAR.
  • Hypertension was a significant risk factor (OR = 5.304), while pre-operative Hb level was a protective factor (OR = 0.909).

Conclusions:

  • Early post-EVAR Hb decrease requires clinical attention, especially in patients with hypertension or low pre-operative Hb.
  • AAA volume loss contributes to post-EVAR Hb decline.
  • Pre-operative Hb level serves as a protective factor against post-EVAR anemia.
