Primary prostatic stromal sarcoma, a rare cancer, was identified in a young man using 18F-PSMA PET/CT. Treatment led to decreased PSMA expression and disappearance of liver lesions.

- Serum prostate-specific antigen (PSA) levels are typically normal in these cases, complicating early diagnosis.

- A case study involving a 23-year-old male with primary prostatic stromal sarcoma is presented.
- 18F-prostate-specific membrane antigen (PSMA) Positron Emission Tomography/Computed Tomography (PET/CT) revealed a prostate mass and multiple liver lesions with high PSMA uptake.
Findings:
- Chemotherapy resulted in a reduction of PSMA expression within the primary prostate tumor.
- The previously identified PSMA-avid liver lesions resolved following treatment.
Implications:
- This case highlights the utility of 18F-PSMA PET/CT in diagnosing and monitoring rare prostatic cancers.
- The findings suggest that PSMA expression can be a responsive biomarker to chemotherapy in prostatic stromal sarcoma.
