Background:

  • C-X-C motif chemokine receptor 4 (CXCR4) imaging offers potential for marginal zone lymphoma (MZL) assessment.
  • Evaluating interobserver agreement is crucial for standardizing CXCR4-targeted PET/CT interpretation.

Purpose of the Study:

  • To determine the interobserver agreement rate of CXCR4-targeted PET/CT in patients with MZL.
  • To assess the impact of reader experience on the reliability of CXCR4-PET/CT interpretation.

Main Methods:

  • 50 MZL patients underwent CXCR4-targeted PET/CT.
  • Four readers (2 experienced, 2 less experienced) evaluated 8 imaging parameters.
  • Intraclass correlation coefficients (ICCs) were used to quantify agreement.

Main Results:

  • Fair agreement (ICC 0.40-0.59) was found for scan results, CXCR4 density, extranodal involvement, and lymph node involvement.
  • Good agreement (ICC 0.71-0.73) was observed for lymph node metastases and areas.
  • Experienced readers showed higher agreement rates in 50% of assessed parameters.

Conclusions:

  • CXCR4-targeted PET/CT demonstrates fair to good interobserver agreement for MZL assessment.
  • Reader experience significantly influences the accuracy of imaging interpretation.
  • Standardized training and experienced personnel are recommended for reliable CXCR4-PET/CT reads.
