Background:

  • Primary Progressive Aphasia (PPA) diagnosis can be challenging.
  • Dysfluency in PPA is often characterized by abnormal pausing, with limited focus on temporal speech features.

Purpose of the Study:

  • To compare speech-related fluency parameters across the three main variants of PPA (logopenic, non-fluent, semantic) and healthy controls.
  • To investigate the utility of temporal speech parameters in characterizing and differentiating PPA subtypes.

Main Methods:

  • A picture description task was used to assess spontaneous speech in 43 adults (13 lvPPA, 10 nfvPPA, 9 svPPA, 11 controls).
  • 42 speech fluency parameters were computed, including syllable duration, speaking pace, interpausal unit (IPU) duration, and linguistic units per IPU/second.

Main Results:

  • Each PPA variant demonstrated unique abnormal speech characteristics, indicating diverse underlying deficits.
  • 37 out of 42 parameters were useful for characterizing PPA dysfluency, and 35 for distinguishing between variants.
  • No single parameter could differentiate between PPA groups, highlighting the multidimensional nature of dysfluency.

Conclusions:

  • Temporal speech parameters, in addition to pausing, offer a comprehensive understanding of PPA dysfluency.
  • Multiparametric analysis is essential for accurately characterizing and differentiating PPA subtypes due to the complexity of dysfluency.
