Summary

A new Merkel cell carcinoma prognostic score (MCC-PS) uses age and lab results to predict patient outcomes. A score of 4 or more indicates high risk and a poorer prognosis for this aggressive skin cancer.

Background:

  • Merkel cell carcinoma (MCC) is an aggressive skin cancer with a historically poor prognosis.
  • Existing immunotherapies have improved outcomes, but a highly accurate predictive model for MCC is still needed.

Purpose of the Study:

  • To develop and validate a Merkel cell carcinoma prognostic score (MCC-PS) using readily available clinical and laboratory data.
  • To identify key risk factors for predicting MCC patient outcomes.

Main Methods:

  • A retrospective study involving 98 MCC patients was conducted.
  • Data collected included age, neuron-specific enolase (NSE), C-reactive protein (CRP), and components of the Model for End-Stage Liver Disease (MELD) score (creatinine, bilirubin, INR).
  • Statistical analyses included correlation, Kaplan-Meier curves, Cox regression, and time-dependent ROC analysis to develop and assess the MCC-PS.

Main Results:

  • The MCC-PS incorporates elevated CRP (≥5.5 mg/l), elevated NSE (≥22.8 µg/l), MELD score (≥11), and age (≥75 years).
  • A score of 4 or more identified a high-risk group with significantly worse 5-year MCC-specific survival (17.6%) compared to the low-risk group (72%).
  • The MCC-PS demonstrated high accuracy in predicting outcomes, with a 5-year MCC-specific survival AUC of 0.93.

Conclusions:

  • The MCC-PS is the first prognostic score for MCC that accurately predicts outcomes using easily accessible laboratory parameters and patient age.
  • An MCC-PS of 4 or more effectively identifies patients with a high risk of poor prognosis, aiding clinical decision-making.
