Background:

Pyogenic ventriculitis involves inflammation of the cerebral ventricular lining due to infection.
Primary pyogenic ventriculitis is exceptionally rare in adults.

Observation:

A 74-year-old female with hypertension and diabetes presented with symptoms.
The patient was diagnosed with Group B Streptococcus (GBS) primary pyogenic ventriculitis.

Findings:

The case represents a unique instance of adult *Streptococcus agalactiae* primary pyogenic ventriculitis.
Successful treatment was achieved with intravenous (IV) antibiotics.

Implications:

Highlights the potential for GBS to cause primary ventriculitis in adults.
Emphasizes the importance of prompt diagnosis and antibiotic treatment for rare central nervous system infections.
