Summary

Single-task tandem gait (TG) is slower after sport-related concussion (SRC). Dual-task (DT) conditions, particularly the auditory pure switching task (APST) response rate, also aid in predicting SRC diagnosis and clinical decisions.

Background:

  • Sport-related concussion (SRC) diagnosis can be challenging.
  • Tandem gait (TG) is a sensitive measure for detecting neurological impairments.

Purpose of the Study:

  • To evaluate dual-task (DT) conditions during TG for predicting SRC.
  • To determine the utility of single-task (ST) and DT measures in SRC diagnosis.

Main Methods:

  • A cohort study involving 409 NCAA Division I student-athletes (controls) and 21 with SRC.
  • Participants completed TG under ST, DT with serial-7s (SS), and DT with auditory pure switching task (APST) conditions.
  • Gait completion time and behavioral task performance were recorded.

Main Results:

  • ST TG was significantly slower in SRC athletes compared to controls.
  • APST response rate was significantly different between and within groups.
  • DT completion times also showed significant differences related to SRC.

Conclusions:

  • ST TG is a valuable objective marker for SRC.
  • DT paradigms, especially APST response rate, offer complementary information for SRC prediction and clinical assessment.
  • Both ST and DT measures should be considered in clinical evaluations of postural instability post-SRC.
