Summary

Patients with acute posttraumatic headache (PTH) after mild traumatic brain injury (mTBI) are at higher risk for persistent PTH and depression. Early PTH detection can help identify individuals needing monitoring for these conditions.

  • Mild traumatic brain injury (mTBI) can result in persistent psychiatric and somatic symptoms.
  • Posttraumatic headache (PTH) and depression are common sequelae of mTBI.
  • The relationship between acute PTH and later depressive symptoms requires further elucidation.

Purpose of the Study:

  • To establish the association between PTH and depression following mTBI.
  • To determine if immediate PTH presence post-injury predicts future depressive symptoms up to 6 months.

Main Methods:

  • Secondary analysis of the prospective HeadSMART study data.
  • Included 265 adult mTBI patients assessed in the emergency department.
  • Utilized Rivermead Post-Concussion Symptoms Questionnaire for acute PTH (aPTH) and Patient Health Questionnaire-9 for depression at baseline and 1, 3, 6 months.

Main Results:

  • Patients with aPTH within 24 hours were more likely to experience PTH at 1, 3, and 6 months.
  • Acute PTH was associated with more severe depressive symptoms and higher likelihood of clinical depression at all follow-up points.
  • Persistent PTH and depression were significantly correlated with initial PTH presence.

Conclusions:

  • Acute PTH within 24 hours of mTBI is a predictor of persistent PTH and depressive symptoms.
  • Emergency department assessment of aPTH can identify patients at risk for prolonged recovery.
  • Monitoring for PTH and depression is recommended for patients with acute PTH post-mTBI.
