Optical coherence tomography (OCT) can predict fractional flow reserve (FFR) using lumen measurements. Specific cutoffs for minimum lumen area (MLA) and area stenosis (%AS) are crucial, especially for proximal coronary segments.

Background:

  • Fractional flow reserve (FFR) assesses the hemodynamic significance of coronary stenoses.

Purpose of the Study:

  • To evaluate the predictive value of OCT parameters for FFR in coronary artery disease.
  • To determine optimal OCT cutoffs for predicting significant FFR values.

Main Methods:

  • A multicenter, pooled analysis of patient-level data from studies combining OCT and FFR.
  • Inclusion of 502 lesions in 489 patients with both OCT and FFR measurements.
  • Analysis of OCT parameters: minimum lumen area (MLA), percentage area stenosis (%AS), thrombus, and plaque rupture.

Main Results:

  • Significant correlations found between OCT-MLA and FFR (R=0.525) and OCT-%AS and FFR (R=-0.482).
  • MLA <2.0 mm² (AUC, 0.80) and %AS >73% (AUC, 0.73) predicted FFR ≤0.80.
  • For proximal segments, optimal cutoffs were MLA <3.1 mm² (AUC, 0.82) and %AS >61% (AUC, 0.84).

Conclusions:

  • OCT-derived lumen measures (MLA, %AS) can predict FFR.
  • Distinct OCT cutoffs are necessary for proximal coronary segments to accurately predict FFR.
