    Summary

This study on cataract surgery found that using the Terry surgical keratometer during wound closure reduced postoperative corneal astigmatism. More patients achieved astigmatism of 2 diopters or less with this technique.

    Area of Science:

    • Ophthalmology
    • Surgical Techniques
    • Corneal Astigmatism Analysis

    Background:

    • Extracapsular cataract extraction (ECCE) with posterior chamber lens implantation is a common surgical procedure.
    • Managing postoperative corneal astigmatism is crucial for visual outcomes.
    • Surgical technique, including wound management, influences refractive results.

    Purpose of the Study:

    • To describe the surgical technique for wound opening and closure in ECCE.
    • To analyze and discuss final postoperative corneal astigmatism.
    • To evaluate the impact of the Terry surgical keratometer on corneal astigmatism.

    Main Methods:

    • A prospective study involving 720 cases of ECCE.
    • Patients were divided into two groups: Group I (using Terry surgical keratometer) and Group II (not using the keratometer).
    • Corneal astigmatism was measured in the early and late postoperative periods.

    Main Results:

    • One of three surgeons showed statistically significant lower K-readings in Group I compared to Group II.
    • A higher percentage of cases in Group I achieved final corneal astigmatism of 2 diopters or less.
    • The Terry surgical keratometer group demonstrated a trend towards reduced postoperative astigmatism.

    Conclusions:

    • The use of the Terry surgical keratometer in ECCE may help minimize postoperative corneal astigmatism.
    • Surgical technique and instrumentation play a role in refractive outcomes after cataract surgery.
    • Further investigation into factors influencing final corneal astigmatism is warranted.
