Background:

  • Sustaining physical activity is challenging; health literacy is crucial for long-term behavior change.
  • Previous research suggests nudges are difficult to implement for sustained physical activity.

Purpose of the Study:

  • To investigate the impact of a commitment-based health application on employee step count.
  • To assess the effect of the application on participants' health literacy levels.

Main Methods:

  • A one-month control experiment involving 109 employees from 7 Japanese companies.
  • Three groups: commitment app (team-based goal setting and reporting), self-commitment (individual goal setting), and control (no intervention).
  • Measured changes in daily step count and health literacy.

Main Results:

  • The commitment app group showed a significant increase of 893 steps compared to the control group.
  • Significant improvements in health literacy were observed in 4 out of 5 measures for the app group.
  • The self-commitment group showed a smaller increase in steps and less significant health literacy improvement.

  • Commitment-based applications with team communication (social nudges) can effectively increase physical activity.
  • App-facilitated social interaction, including sharing achievements and encouragement, enhances behavioral reinforcement.
  • Improved health literacy is a potential pathway for long-term maintenance of physical activity behaviors.
