A history of endometriosis increases the risk of placenta previa, a serious pregnancy complication. Complete surgical treatment for advanced endometriosis before pregnancy may reduce this risk.
Area of Science:
- Reproductive Medicine
- Obstetrics
- Gynecologic Surgery
Background:
- Endometriosis is increasingly linked to adverse perinatal outcomes.
- The impact of pre-pregnancy endometriosis treatment on these complications remains unclear.
- This study investigates the association between endometriosis and placenta previa, considering surgical intervention.
Purpose of the Study:
- To determine if a history of endometriosis is an independent risk factor for placenta previa.
- To assess the influence of surgical endometriosis treatment on placenta previa incidence.
Main Methods:
- A case-control study of 2781 deliveries (133 with endometriosis, 2648 without).
- Comparison of perinatal outcomes using statistical tests (t-test, Fisher's exact test).
- Logistic regression analysis to identify risk factors for placenta previa and evaluate surgical impact.
Main Results:
- A history of endometriosis significantly elevates the risk of placenta previa (adjusted OR, 2.30).
- Complete surgical treatment for advanced (Stage III-IV) endometriosis was associated with a lower placenta previa incidence (5.9% vs. 33.3%).
Conclusions:
- Endometriosis history is an independent risk factor for placenta previa.
- Complete surgical management of advanced endometriosis may mitigate placenta previa risk.
- Further research is warranted to confirm the benefits of endometriosis surgery on perinatal complications.
