Evaluation indicators for access to cancer screening services: a scoping review
1Office of Cancer Screening, National Cancer Center/National Clinical Research Center for Cancer/Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College, Beijing 100021, China.
This study reviewed 331 publications on cancer screening access, identifying key indicators. The findings will help develop a better system for evaluating cancer screening access and its availability.
Area of Science:
- Health Services Research
- Cancer Epidemiology
- Public Health
Background:
- Access to healthcare services, particularly cancer screening, is crucial for effective public health strategies.
- The International Agency for Research on Cancer (IARC) emphasizes 'availability' as a key indicator for cancer screening.
- Current methods for evaluating and reporting cancer screening access indicators are inconsistent, hindering evidence-based decision-making.
Purpose of the Study:
- To systematically identify and collate existing indicators used to evaluate access to cancer screening services.
- To understand the current application and gaps in the use of these indicators.
- To inform the development of a standardized evaluation system for cancer screening access.
Main Methods:
- A comprehensive scoping review of studies published between 2013 and 2022 was conducted.
- Indicators of access to cancer screening services were extracted and quantified.
- Extracted indicators were mapped to two established frameworks: the universal five-dimensional framework for healthcare access (U5D) and the IARC's cancer-specific framework.
Main Results:
- A total of 331 studies were included in the review.
- Analysis revealed various indicators across supply-side (e.g., approachability, availability) and demand-side (e.g., ability to perceive, seek, reach, pay) perspectives.
- IARC-endorsed indicators related to policy, screening type, coverage, and demographics were also identified, leading to the proposal of an integrated framework.
Conclusions:
- This review successfully identified and organized a wide range of indicators for assessing cancer screening access.
- Significant gaps in the current application of these indicators were determined.
- The findings provide a foundation for developing a robust and systematic indicator system to improve cancer screening evaluation and accessibility.
