Background:

  • Military fighter aircrew experience high rates of neck pain and injury.
  • A standardized 8-week Spine Training Program (STP) is implemented at Eglin and Luke Air Force Bases for student pilots (SPs).
  • There is no established consensus on optimal training methods for mitigating cervical injuries in this population.

Purpose of the Study:

  • To assess the impact of the STP on Cervical Endurance Hold (CEH) performance in SPs.
  • To determine if the training effects on CEH are consistent across different training locations (Eglin AFB vs. Luke AFB).
  • To test the hypothesis that SPs show significant CEH improvements regardless of training base.

Main Methods:

  • A retrospective analysis of 198 F-16 and F-35 SPs enrolled in the Basic Course at Luke AFB and Eglin AFB from 2020-2023.
  • The standardized 8-week STP was administered, with CEH tests conducted pre- (intake) and post- (exit) training.
  • Statistical analysis using SPSS, excluding participants unable to complete CEH tests at intake or exit.

Main Results:

  • All participant groups showed statistically significant improvements in CEH from intake to exit (P < 0.001).
  • Overall, a 33.6% increase in CEH was observed across all participants, with a large effect size (d = 1.14).
  • Significant CEH increases were noted in all subgroups: Luke AFB females (20.4%), Luke AFB males (34.5%), and Eglin AFB males (55.7%).

Conclusions:

  • The STP resulted in significant improvements in Cervical Endurance Hold across all studied groups.
  • The training's effectiveness, indicated by a large effect size, was consistent regardless of the training location.
  • Further research, including a randomized controlled trial comparing different STPs, is recommended to optimize fighter aircrew neck injury prevention.
