Summary

Pulsed-field ablation (PFA) and cryoballoon ablation (CBA) show similar long-term pulmonary vein isolation (PVI) durability. Repeat ablation procedures reveal comparable electrical reconduction rates and patterns between PFA and CBA, indicating equivalent efficacy for PVI.

Background:

  • Pulsed-field ablation (PFA) is a novel technology for pulmonary vein isolation (PVI), with initial studies showing promise.
  • Long-term data on PVI durability and reconduction patterns following PFA are limited compared to established methods like cryoballoon ablation (CBA).

Purpose of the Study:

  • To compare the long-term durability and reconduction patterns of PVI achieved with PFA versus CBA.
  • To analyze findings from repeat ablation procedures in patients who previously underwent PFA or CBA for PVI.

Main Methods:

  • A total of 550 patients undergoing index PVI via PFA or CBA were analyzed.
  • Repeat ablation procedures in patients with recurrent symptomatic atrial arrhythmias were evaluated.
  • Electrical reconduction of pulmonary veins (PVs) was assessed using multipolar spiral mapping and 3D mapping.

Main Results:

  • Repeat ablation was performed in 12% of patients after PFA and 12% after CBA.
  • Electrical reconduction was detected in 73% of PVs after PFA and 75% after CBA (P=1.000).
  • Rates of reconduction per initially isolated PV were similar: 38% for PFA and 37% for CBA (P=0.936).

Conclusions:

  • Electrical reconduction rates and patterns of pulmonary veins are similar after repeat ablation procedures following either PFA or CBA.
  • Both PFA and CBA demonstrate comparable long-term durability for PVI based on repeat ablation findings.
