Rous Sarcoma virus or RSV was discovered by F. Peyton Rous in the year 1911 as a filterable transmissible agent that could cause tumors in chickens. He won a Nobel Prize for this discovery in 1966. Background:

Perianal embryonal rhabdomyosarcoma (ERMS) is a rare pediatric malignancy with a poor prognosis.
Limited reported cases and nonspecific clinical signs present diagnostic challenges.

Observation:

A 30-year-old male presented with severe perianal pain, initially diagnosed as a perianal abscess via ultrasonography.
Emergency incision and drainage were performed, but subsequent histopathology and immunohistochemistry confirmed ERMS.

Findings:

The patient, untreated with adjuvant therapy, developed widespread metastases and died within six months.
Perianal ERMS is highly malignant and frequently misdiagnosed as a perianal abscess.

Implications:

Enhanced clinician awareness and improved preoperative diagnostic tools are essential to prevent misdiagnosis of perianal ERMS.
This case highlights the critical need for vigilance in diagnosing rare perianal malignancies.
