Background:

  • Coronavirus disease-2019 (COVID-19) survivors may experience persistent symptoms and lung abnormalities post-recovery.
  • This condition is termed post-COVID interstitial lung disease (ILD) or residual disease.

Purpose of the Study:

  • To evaluate the characteristics, risk factors, and clinical significance of post-COVID ILD.
  • To define diagnostic criteria for post-COVID ILD.

Main Methods:

  • Multicenter cross-sectional analysis of data from a randomized clinical study.
  • Patients with persistent respiratory symptoms 3 months post-COVID-19 were assessed.
  • Post-COVID ILD defined by symptoms, hypoxemia, restrictive lung function, and HRCT findings.

Main Results:

  • 262 out of 375 patients met criteria for post-COVID ILD.
  • Dyspnea, exercise intolerance, and fatigue were common symptoms.
  • Ground glass opacities and fibrotic bands were frequent HRCT findings.
  • Active smoking, ICU admission, and elevated LDH were risk factors for post-COVID ILD.

Conclusions:

  • Post-COVID ILD is characterized by respiratory symptoms, radiological lesions, and restrictive lung defects.
  • Active smoking, severe COVID-19, and elevated LDH are significant risk factors.
  • Approximately 20% of patients experience spontaneous improvement in symptoms and radiological findings.
