Summary

Forward head posture (FHP) significantly impacts gait when performing cognitive tasks. Individuals with FHP showed altered gait parameters and increased cognitive cost during dual-task walking compared to those with normal head posture.

Background:

  • Forward head posture (FHP) is linked to physiological changes.

Purpose of the Study:

  • To determine if forward head posture affects gait parameters during cognitive challenges.
  • To compare gait kinematics between individuals with FHP and normal head posture (NHP) under single-task and dual-task conditions.

Main Methods:

  • Forty-five participants with FHP (craniovertebral angle [CVA] < 50°) were matched with 45 participants with NHP (CVA > 55°).
  • Gait kinematics were measured using optical motion capture during single-task and dual-task walking.
  • Dual-task cost (DTC) was calculated to assess the impact of cognitive load on gait.

Main Results:

  • No significant gait differences were observed between FHP and NHP groups under single-task conditions.
  • In the dual-task condition, 12 out of 13 gait parameters significantly differed between groups (p < 0.01).
  • Participants with FHP exhibited significantly increased DTC for all gait parameters (p < 0.02), with significant correlations between CVA and gait/DTC under dual-task conditions (p < 0.01).

Conclusions:

  • Forward head posture significantly elevates the cognitive cost associated with walking.
  • Proper head and neck alignment is crucial for maintaining gait stability and cognitive function during dual-task activities.
  • These findings underscore the clinical relevance of addressing FHP to optimize functional mobility and cognitive performance.
