Background:

  • Sarcopenia is a recognized comorbidity in heart failure, negatively impacting patient prognosis.
  • Low muscle strength is prevalent in elderly patients with acute heart failure (AHF).

Purpose of the Study:

  • To identify differential characteristics of polypathological patients with AHF based on muscle strength.
  • To determine if low muscle strength is associated with a worse prognosis in AHF patients.

Main Methods:

  • An observational study of 377 AHF patients from the PROFUND-IC registry.
  • Low muscle strength assessed via dynamometry or handgrip strength.
  • Analysis included epidemiological data, comorbidities, AHF etiology, prior admissions, NYHA class, functionality scores, and mortality/readmission rates.

Main Results:

  • 82.23% of patients (mean age 83) exhibited low muscle strength.
  • Patients with low muscle strength were older, had more cognitive impairment, worse functional class, and higher PROFUND index scores.
  • While readmission and mortality rates were higher in the low muscle strength group, differences were not statistically significant.

Conclusions:

  • Elderly polypathological patients with AHF and low muscle strength present with a higher PROFUND index.
  • Low muscle strength is associated with a reduced probability of annual survival in this patient population.
