    Summary

Nitroderm TTS (transdermal therapeutic system) and isosorbide dinitrate improve exercise tolerance in patients with coronary heart disease. This new nitroglycerin system offers convenient once-daily dosing.

    Area of Science:

    • Cardiology
    • Pharmacology
    • Biomedical Engineering

    Background:

    • Coronary heart disease and exercise-induced angina pectoris remain significant clinical challenges.
    • Nitroglycerin is a key vasodilator for managing angina.
    • Transdermal drug delivery systems offer potential for sustained and controlled medication release.

    Purpose of the Study:

    • To evaluate the efficacy and tolerability of Nitroderm TTS (transdermal therapeutic system) for nitroglycerin delivery.
    • To compare Nitroderm TTS 10 mg/24h with placebo and isosorbide dinitrate (ISDN) 20 mg slow-release in patients with stable exercise-induced angina.

    Main Methods:

    • A randomized, double-blind, within-patient study using a 3x3 Latin square design.
    • Nine in-patients with coronary heart disease and stable exercise-induced angina participated.

    Related Experiment Videos

  • Treatments included Nitroderm TTS 10 mg/24h, ISDN 20 mg slow-release, and placebo, administered over 3 successive days.
  • Main Results:

    • Both Nitroderm TTS 10 and ISDN significantly reduced systolic blood pressure at rest compared to placebo.
    • Exercise tolerance on a symptoms-limited cycloergometric test improved significantly with both TTS 10 and ISDN at 4 hours post-dosing.
    • No significant difference in efficacy was observed between Nitroderm TTS 10 and ISDN.

    Conclusions:

    • Nitroderm TTS 10 mg/24h and ISDN 20 mg are effective in improving exercise tolerance in patients with exercise-induced angina.
    • The transdermal therapeutic system provides a consistent plasma nitroglycerin level over 24 hours.
    • Nitroderm TTS offers the advantage of once-daily administration for managing angina pectoris.
