Blinding is a commonly used method of not telling participants which treatment a subject is receiving. Blinding is a critical part of a randomized control trial or RCT. It reduces the bias that affects the results. In an RCT, blinding is used in the form of a placebo. A placebo effect occurs when untreated subjects falsely believe they have received the treatment and report improved symptoms. A placebo or a dummy treatment is administered to subjects to negate the bias caused by such an effect.

Purpose of the Study:

  • To assess the impact of very low nicotine content (VLNC) cigarettes on biomarkers of exposure in Black and White smokers.
  • To determine if reduced nicotine cigarette efficacy differs by race.

Main Methods:

  • Pooled data from two randomized controlled trials comparing VLNC cigarettes to usual nicotine content (UNC) cigarettes over 18 weeks.
  • Utilized linear regression and intention-to-treat (ITT) analysis, including biomarker data (plasma cotinine, exhaled carbon monoxide).
  • Conducted a secondary compliance analysis focusing on participants exclusively using VLNC cigarettes.

Main Results:

  • Both Black and White smokers using VLNC cigarettes showed significant reductions in plasma cotinine and exhaled carbon monoxide compared to UNC groups.
  • No significant treatment-by-race interaction was observed for most outcomes.
  • A marginal interaction for cotinine reduction in the ITT analysis indicated slightly greater reduction in White smokers.

Conclusions:

  • Reduced nicotine cigarette regulations can substantially decrease nicotine exposure and toxicant intake for both Black and White smokers.
  • Findings support the equitable application of reduced nicotine standards across racial groups.
