    Background:

    • Nephrotic syndrome (NS) diagnosis has advanced with renal biopsy, electron microscopy, and immunohistology.
    • Two primary categories of NS are identified: those with diffuse glomerular lesions and idiopathic NS (INS).

    Purpose of the Study:

    • To differentiate clinico-pathological conditions associated with nephrotic syndrome.
    • To clarify the diagnostic criteria and prognostic indicators for various forms of NS.

    Main Methods:

    • Clinical symptom analysis (hematuria, hypertension, proteinuria selectivity).
    • Renal biopsy findings (diffuse glomerular lesions, minimal change NS, diffuse mesangial proliferation, focal glomerular sclerosis).
    • Immunohistologic and electron microscopic examination.
    • Response to steroid therapy.

    Main Results:

    • Immune-related glomerulopathies often present with moderate NS, hematuria, hypertension, and steroid resistance.
    • Idiopathic NS (INS), predominantly minimal change NS (MCNS), shows marked NS, selective proteinuria, and good steroid response.
    • Diffuse mesangial proliferation (DMP) and focal glomerular sclerosis are associated with poorer prognoses and steroid resistance.

    Conclusions:

    • Renal biopsy is crucial for NS diagnosis, particularly when clinical signs suggest diffuse glomerular disease or steroid resistance.
    • MCNS, DMP, and focal glomerular sclerosis may represent variants of the same disease rather than distinct entities.
    • Congenital NS of Finnish type and infantile mesangial sclerosis are distinct NS forms with poor prognoses in infancy.
