Background:

  • Uncontrolled human immunodeficiency virus (HIV) infection can lead to diverse hematologic abnormalities.
  • These abnormalities arise from direct viral effects, opportunistic infections, malignancy, or therapeutic interventions.
  • Patients with HIV are inherently susceptible to bone marrow dysfunction.

Observation:

  • A patient with uncontrolled HIV presented with acute, severe thrombocytopenia.
  • This complication occurred shortly after initiating treatment for disseminated Mycobacterium avium complex (MAC).
  • Thrombocytopenia was refractory to platelet transfusions and intravenous immunoglobulin (IVIG).

Findings:

  • Thrombocytopenia showed mild improvement with dexamethasone after withholding home medications.
  • Potential etiologies include uncontrolled HIV and medication-induced thrombocytopenia, possibly secondary to rifabutin.
  • A combined effect of HIV and rifabutin is proposed as the cause.

Implications:

  • Clinicians must recognize the heightened risk of severe, acute medication-induced thrombocytopenia in HIV patients.
  • Awareness is crucial for managing hematologic complications in this vulnerable population.
  • Prompt identification and management of drug-induced thrombocytopenia are essential for patient outcomes.
