Methods of Documentation VI: Case Management Model

564
The case management model is a multidisciplinary approach that involves healthcare professionals from diverse disciplines, such as physicians, nurses, therapists, social workers, and pharmacists, working collaboratively to address the various needs of patients. Each healthcare professional brings unique expertise and perspectives, contributing to a more comprehensive understanding of the patient's condition and tailoring treatment plans accordingly.
For example, a patient with a chronic...

Data Collection I01:30

Data Collection I

6.1K
Data collection gathers information needed to make accurate judgments about a patient's present condition. During a health history interview, subjective data is collected from the patient, their caregivers, or family members, and objective data is collected through observations and physical assessment. Patients are the primary source of subjective data. Thus information gathered from patients through interviews, observations, and physical examination is primary data. Secondary sources of...

Documentation in Long-Term and Home Healthcare Setting

875
Documentation in long-term care facilities and home healthcare settings is crucial for ensuring continuous, coordinated, and comprehensive care for patients. Each setting has its specific documentation processes and tools:

The nursing history captures and records the patient's health status, so that a care plan evolves to meet the patient's individual needs. The nursing health history is a part of the initial assessment. A comprehensive history covers all health dimensions and plays a significant role in the assessment process. A comprehensive history includes the patient's biographical information, reasons for seeking health care, expectations, present and past health history, medications, and...

Patient-centered Care01:13

Patient-centered Care

2.0K
Patient-centered care involves delivering care beyond inpatient hospitalization. Reflective practice can enhance a patient-centered approach. Reflective practice is a process of reasoning that considers all aspects of the present situation, including practicalities, learning from personal practice, and consideration of patient needs. Patients appreciate care decisions made while considering their input. Involving the patient in their care provides the patient with a sense of contribution rather...

Methods of Documentation IV: Focus Charting

1.0K
Focus Charting, also known as the focus charting system or "focus documentation," is a systematic documentation approach used in healthcare to organize patient information in medical records.
It typically involves three columns for recording information:

Area of Science:

  • Digital Health
  • Human-Computer Interaction
  • Health Communication

Background:

  • Patient-centered care models emphasize patient involvement in healthcare.
  • Mobile health (mHealth) is key to patient-centered care but faces adoption barriers.
  • Current mHealth apps often lack patient engagement and provider workflow integration.

Purpose of the Study:

  • To investigate changes in patient-provider communication behaviors when patient-generated data is introduced.
  • To evaluate the impact of patient-generated data on patient-centered communication.

Main Methods:

  • Design science approach used to develop PatientHub, an integrated digital health system.
  • PatientHub was iteratively developed and evaluated in a 3-week field study.
  • Analysis of 54 video-recorded consultations and interviews with 27 patients and 6 physicians.

Main Results:

  • Introduction of patient-generated data into consultations yielded three emergent behaviors: emotion labeling, expectation decelerating, and decision ping-pong.
  • These behaviors were found to enhance patient-provider communication, making consultations more personal, actionable, trustworthy, and equal.
  • Patient-generated data actively engages both patients and providers in the care process.

Conclusions:

  • Patient-generated data significantly facilitates patient-centered care by enhancing patient and provider engagement.
  • Three design principles for patient-centered communication were proposed.
  • Findings offer insights for designing future mHealth systems and understanding mHealth-supported communication in chronic care.
