Summary

Lacertus syndrome, a rare cause of median nerve compression, can cause finger numbness and weakness. Surgical release of the lacertus fibrosus effectively resolved symptoms in a surgeon, confirming its diagnostic utility.

Background:

Median nerve entrapment can present with varied symptoms, including finger numbness and weakness.
Lacertus syndrome, a less common cause of median nerve compression, involves the lacertus fibrosus.
Standard treatments like corticosteroid injections and splinting may not always resolve symptoms.

Observation:

A 48-year-old surgeon experienced progressive finger numbness, pain, cramps, and weakness for two years.
Initial treatments for carpal tunnel syndrome were ineffective.
A positive sensory collapse test, though atypical, prompted further investigation.

Findings:

Combined sensory collapse test and electromyography revealed transient, partial muscle tone collapse upon cutaneous stimulation.
Surgical release of the median nerve at the lacertus fibrosus successfully resolved the patient's symptoms.
Post-surgery, the combined sensory collapse test and electromyography showed minimal to no muscle tone collapse.

Implications:

This case highlights the importance of considering lacertus syndrome in cases of persistent median nerve symptoms.
The modified sensory collapse test combined with electromyography may aid in diagnosing lacertus syndrome.
Surgical decompression of the median nerve at the lacertus fibrosus is an effective treatment for this condition.
