    Summary

Preputial abscesses in men were linked to Neisseria gonorrhoeae infections acquired through sexual contact in Thailand. Many strains showed resistance to common antibiotics, highlighting a growing public health concern.

    Area of Science:

    • Urology
    • Infectious Diseases
    • Microbiology

    Background:

    • Neisseria gonorrhoeae is a significant cause of sexually transmitted infections.
    • Antibiotic resistance in Neisseria gonorrhoeae is a growing global health challenge.
    • Preputial abscesses are a potential, though less common, manifestation of urogenital infections.

    Observation:

    • Three cases of preputial abscesses were observed in men following sexual contact with prostitutes in Thailand.
    • Neisseria gonorrhoeae was successfully cultured from the abscess lesions in all three patients.
    • Despite the absence of clinical urethritis symptoms, urethral swabs were positive for gonococci in two of the three patients.

    Findings:

    • Two of the isolated Neisseria gonorrhoeae strains exhibited beta-lactamase production, indicating resistance to beta-lactam antibiotics.

    Related Experiment Videos

  • The presence of Neisseria gonorrhoeae in preputial abscesses suggests a direct inoculation or spread from an infected site.
  • Asymptomatic urethral infections co-existed with symptomatic preputial abscesses in some cases.
  • Implications:

    • This suggests Neisseria gonorrhoeae can cause extragenital infections like preputial abscesses.
    • The findings underscore the importance of considering antibiotic resistance patterns in Neisseria gonorrhoeae, particularly beta-lactamase producing strains.
    • Healthcare providers should maintain a high index of suspicion for gonococcal infections in patients presenting with preputial abscesses, even without typical urethritis symptoms.
