Functional Classification of Joints

Functional Classification of Joints
The functional classification of joints is determined by the amount of mobility between the adjacent bones. Joints are functionally classified as a synarthrosis or immobile joint, an amphiarthrosis or slightly moveable joint, or as a diarthrosis, a freely moveable joint. Fibrous and cartilaginous joints can be functionally classified as either synarthroses or amphiarthroses, whereas all synovial joints are classified as diarthroses.

Summary

Medication patterns before total hip replacement (THR) surgery vary significantly among coxarthrosis patients. Many patients show low medication use and do not exhaust conservative treatments before THR.

Background:

Geographical variations in total hip replacement (THR) rates and treatment guideline adherence are documented.
Understanding patient healthcare trajectories and medication use before THR is crucial for optimizing treatment.

Purpose of the Study:

To identify and visualize medication use patterns in coxarthrosis patients prior to surgery.
To examine the association of these patterns with patient characteristics and THR.
To compare observed medication use with treatment guidelines for analgesics and conservative therapy.

Main Methods:

State Sequence Analysis (SSA) applied to German health insurance data (2012-2015).
Analysis of coxarthrosis patients, comparing those who underwent THR with those who did not within a 1-year period.
Clustering of medication use patterns based on prescription data and logistic regression to assess associations with THR.

Main Results:

Seven distinct medication use patterns identified, strongly correlating with patient characteristics and THR.
Two patterns indicated exhaustion of pharmacological therapy preceding THR.
Opioid use was limited to specific patterns with lower odds of THR; the most common pattern showed minimal pharmacological therapy.

Conclusions:

Significant heterogeneity exists in pre-surgical medication use patterns for coxarthrosis patients.
While some adherence to stepped care and cautious opioid use was observed, many patients did not demonstrate exhaustive conservative treatment before THR.
