Magnetic resonance imaging (MRI) is a noninvasive medical imaging technique based on a phenomenon of nuclear physics discovered in the 1930s, in which matter exposed to magnetic fields and radio waves was found to emit radio signals. Summary

Apparent diffusion coefficient (ADC) values from diffusion-weighted imaging (DWI) can help evaluate gallbladder tumors. Lower ADC values indicate malignancy and poorer prognosis in gallbladder cancer patients.

Purpose of the Study:

  • To investigate the utility of apparent diffusion coefficient (ADC) values in evaluating malignancy and prognosis of gallbladder tumors.
  • To correlate ADC values with clinicopathological factors in gallbladder cancer.

Main Methods:

  • Retrospective analysis of 63 patients (33 benign, 30 cancer) who underwent surgical resection for gallbladder tumors.
  • Acquisition of mean ADC values using DWI-MRI.
  • Comparison of ADC values between benign and malignant tumors, and early vs. advanced gallbladder cancer.
  • Stratification of advanced gallbladder cancer cases into ADC-High and ADC-Low groups for survival analysis.

Main Results:

  • Significantly lower ADC values were observed in advanced and early gallbladder cancer compared to benign tumors (P<0.05).
  • The ADC-Low group in advanced gallbladder cancer showed a trend towards higher rates of advanced stage disease (P=0.09).
  • Disease-free survival and overall survival (OS) were significantly worse in the ADC-Low group compared to the ADC-High group (P<0.01).
  • Multivariate analysis identified poor differentiation and low ADC value as independent prognostic factors for OS.

Conclusions:

  • ADC values derived from DWI-MRI show potential for differentiating malignant from benign gallbladder tumors.
  • Low ADC values are associated with poorer prognosis and advanced disease in gallbladder cancer.
  • ADC measurements may serve as a valuable non-invasive biomarker for gallbladder tumor evaluation.
