    Background:

    • Annulo-aortic ectasia (AAE) is a serious condition affecting the ascending aorta.
    • Understanding the histological changes in AAE is crucial for patient management.

    Purpose of the Study:

    • To investigate the microscopic and ultrastructural changes in the aortic media of patients with AAE.
    • To compare these changes between patients with and without Marfan syndrome.

    Main Methods:

    • Light and electron microscopy were used to examine aortic wall specimens from 44 patients with AAE.
    • Comparison was made with control autopsy specimens.

    Main Results:

    • Patients with AAE showed significantly increased cystic change, elastic fragmentation, fibrosis, and smooth muscle cell loss compared to controls.
    • Ultrastructural findings included proteoglycan pooling, elastic lamellae degeneration, increased collagen, and smooth muscle cell necrosis.
