Abstract

Insights

The Plasminogen activator inhibitor-1 (PAI-1) 4G/4G genotype is linked to greater residual thrombus in mesenteric venous thrombosis (MVT) patients. This PAI-1 4G/4G genotype predicts persistent thrombus after the acute phase of MVT.

Area of Science:

  • Genetics and Thrombosis
  • Vascular Medicine

Background:

  • Plasminogen activator inhibitor-1 (PAI-1) regulates fibrinolysis.
  • The PAI-1 4G/5G gene polymorphism may influence PAI-1 activity and coagulation-fibrinolysis balance.

Purpose of the Study:

  • To investigate the association between PAI-1 4G/5G genotype and the development and residual thrombus in acute primary mesenteric venous thrombosis (MVT).

Main Methods:

  • Retrospective review of 34 acute primary MVT patients.
  • Fluorescence in situ hybridization to determine PAI-1 4G/5G polymorphism.
  • Stratification of patients based on PAI-1 genotype.

Main Results:

  • The 4G/4G genotype was observed in 32.3% of patients, and non-4G genotypes (4G/5G, 5G/5G) in 67.6%.
  • No correlation was found between PAI-1 genotype and the initial extent of thrombosis.
  • Patients with the 4G/4G genotype showed a significantly larger thrombus burden and less thrombus resolution at follow-up (p < 0.05).

Conclusions:

  • The PAI-1 4G/4G genotype is a predictor of residual thrombus in mesenteric veins following the acute phase of MVT.
