    Summary

    Background:

    • Cefminox (CMNX) is a novel cephamycin antibiotic developed for treating bacterial infections.
    • Cephamycins are a subclass of beta-lactam antibiotics known for their stability against beta-lactamases.

    Purpose of the Study:

    • To evaluate the in vitro antimicrobial activity of cefminox (CMNX) against various clinical isolates.
    • To assess the clinical efficacy and safety of cefminox (CMNX) in patients with bacterial infections.

    Main Methods:

    • Minimum Inhibitory Concentrations (MICs) were determined for CMNX against clinical bacterial isolates using an inoculum size of 10(6) cells/ml.
    • Clinical efficacy was assessed in five patients with pneumonia, pneumonia and sepsis, or urinary tract infections, treated with CMNX (1-4 g daily for 7-31 days).

    Main Results:

    • CMNX demonstrated high susceptibility rates (>80%) against E. coli, K. pneumoniae, P. mirabilis, and indole-positive Proteus sp., with MICs typically between 0.39-0.78 µg/ml.
    • Clinical treatment showed variable responses: excellent in 1, good in 3, and poor in 2 patients. Bacteriological effects were good for E. coli, K. pneumoniae, and S. liquefaciens, but poor for P. aeruginosa and P. mirabilis.
    • No adverse side effects or abnormal laboratory findings were reported during the clinical study.

    Conclusions:

    • Cefminox (CMNX) exhibits potent in vitro activity against a range of Gram-negative bacteria, including key pathogens like E. coli and K. pneumoniae.
    • Clinical application of CMNX showed promising results in some infections, though efficacy varied. Further investigation may be warranted.
    • Cefminox (CMNX) appears to be well-tolerated, with a favorable safety profile observed in the limited clinical cases studied.
