Background:

  • Lung transplantation (LTx) is a critical intervention for end-stage lung disease.
  • Understanding post-transplant cancer risk and outcomes in patients with prior cancer history is crucial.

Purpose of the Study:

  • To determine the incidence of malignancies after lung transplantation.
  • To evaluate the long-term survival of lung transplant recipients with a pre-transplant cancer diagnosis.

Main Methods:

  • Retrospective cohort study including 614 lung transplant recipients (1990-2016).
  • Cancer incidence determined via linkage with the Swedish Cancer Registry and Cause-of-Death registry.
  • Standardized Incidence Ratios (SIR) calculated to compare cancer risk with the general population.

Main Results:

  • 159 malignancies diagnosed post-LTx, a 5.6-fold increased risk compared to the general population.
  • Most common post-LTx cancers: non-melanoma skin cancer (SIR 76.5), non-Hodgkin lymphoma (SIR 23.5), and lung cancer (SIR 8.89).
  • No significant difference in overall survival between recipients with and without prior cancer history (p=0.56).

Conclusions:

  • Lung transplant recipients have a substantially elevated risk of developing subsequent cancers.
  • A prior cancer diagnosis does not appear to adversely affect long-term survival in carefully selected lung transplant candidates.
