Background:

Prescribing errors (PEs) are a major global health concern.
Limited research exists on the impact of pharmacist staffing on PEs in Malaysia.
This study investigates the relationship between pharmacist presence and intervention effectiveness in Malaysian healthcare facilities.

Purpose of the Study:

To analyze intervention trends related to PEs in Malaysia.
To assess the association between pharmacist staffing levels and intervention frequency/effectiveness.
To highlight the correlation between increased pharmacist presence and the number of interventions on PEs.

Main Methods:

Retrospective cross-sectional study using data from 2017-2019.
Analysis of Ministry of Health Pharmacy Management Forms and Pharmacy Board Registry.
Multivariate regression and two-way ANOVA to assess associations between pharmacist numbers, prescriptions, and interventions.

Main Results:

Pharmacists intervened in approximately 1.8% of total prescriptions annually.
Common errors included wrong dose, wrong medication, and wrong dosing frequency.
A significant positive correlation was found between the number of pharmacists, total prescriptions, and interventions (adjusted R-squared = 0.899).

Conclusions:

Increased pharmacist numbers and prescription volume are key predictors of PEs interventions in Malaysia.
Pharmacists play a vital role in enhancing patient safety and healthcare quality.
Effective pharmacist interventions are consistent across various healthcare settings.
