Summary

Genetic testing for prostate cancer (PCa) is underutilized in India, with low prescription rates for homologous recombination repair (HRR) genes. Improving affordability and access to testing and targeted therapies is crucial for personalized PCa care.

Area of Science:

  • Oncology
  • Genetics
  • Medical Practice

Background:

  • International guidelines recommend genetic testing for targeted therapy selection in prostate cancer (PCa).
  • Limited data exists on current genetic testing practices for PCa within India.

Purpose of the Study:

  • To investigate the current patterns of genetic testing for prostate cancer (PCa) in India.
  • To identify barriers and facilitators for genetic testing in PCa management.

Main Methods:

  • A validated online questionnaire was distributed to oncologists and uro-oncologists across India.
  • Data was collected from February to May 2022, with responses analyzed using descriptive statistics.

Main Results:

  • Only 35.9% of participants advised genetic testing in less than 5% of PCa patients.
  • Homologous recombination repair (HRR) gene testing was routine for 50.2%, with blood and tissue being preferred specimens.
  • Key barriers included cost and lack of genetic counselors; cost-effectiveness and improved medication access were identified as solutions.

Conclusions:

  • A low frequency of genetic testing for HRR genes was observed across India.
  • Enhancing the quality, accessibility, and cost-effectiveness of genetic testing and targeted therapies is essential for personalized metastatic PCa care.
