Redo surgical aortic valve replacement (rSAVR) for structural valve deterioration shows similar outcomes to primary SAVR when accounting for comorbidities. Elective rSAVR offers comparable safety and efficacy to initial valve replacement surgery.
Area of Science:
- Cardiovascular Surgery
- Heart Valve Disease
- Bioprosthetic Valves
Background:
- Structural valve deterioration (SVD) is a common complication requiring reoperation.
- Redo surgical aortic valve replacement (rSAVR) carries potential risks compared to primary surgery.
Purpose of the Study:
- To compare outcomes of primary bioprosthetic surgical aortic valve replacement (SAVR) versus isolated rSAVR due to SVD.
- To identify risk factors and assess the safety of rSAVR.
Main Methods:
- Retrospective comparison of clinical data from patients undergoing primary SAVR and rSAVR (2011-2022).
- Primary outcome: all-cause mortality or stroke during hospitalization.
- Secondary outcomes: myocardial infarction, re-exploration for bleeding, pacemaker implantation.
Main Results:
- A total of 2620 patients were analyzed; 6.6% underwent rSAVR.
- Primary SAVR patients were older and had fewer prior strokes.
- Univariate analysis showed higher mortality/stroke in rSAVR, but multivariable analysis and matched comparisons found no significant difference in adverse outcomes.
Conclusions:
- Initial univariate analysis suggests higher risk for rSAVR, but comorbidities largely explain this.
- Elective rSAVR can achieve outcomes comparable to primary SAVR.
- Patient selection and management of comorbidities are crucial for successful rSAVR.
