    Summary

    This study analyzed epileptic falls in children with Lennox-Gastaut syndrome, finding flexor spasms to be the most common type, often preceded by West syndrome. Atonic falls were less common than expected.

    Background:

    • Epileptic falls, or drop attacks, are a significant challenge in managing pediatric epilepsy syndromes.
    • Lennox-Gastaut syndrome is characterized by multiple seizure types, including falls, impacting quality of life.

    Purpose of the Study:

    • To precisely classify the types of epileptic falls in children diagnosed with Lennox-Gastaut syndrome.
    • To investigate the relationship between fall types, prior epilepsy syndromes, and electroencephalogram (EEG) findings.

    Main Methods:

    • Utilized a self-tracking video monitoring system to capture 48 epileptic falls in 15 children.
    • Defined falls as a rapid loss of postural maintenance (standing or sitting) within 1 second.
    • Classified falls into tonic, flexor spasms, myoclonic-atonic, and atonic types based on precise sequential analysis.

    Main Results:

    • Flexor spasms type falls were most prevalent (25 falls in 8 children), contrary to expectations of atonic or myoclonic-atonic falls being common.
    • Seven of eight children with flexor spasms falls had a history of West syndrome, preceding Lennox-Gastaut syndrome.
    • Tonic falls occurred in 9 instances (5 children), and myoclonic-atonic falls in 12 instances (3 children).

    Conclusions:

    • The classification of epileptic falls in Lennox-Gastaut syndrome reveals a higher incidence of flexor spasms than previously assumed.
    • A history of West syndrome is frequently associated with the flexor spasms type of fall in Lennox-Gastaut syndrome.
    • Highlights the complex pathophysiology underlying various epileptic fall types in children.
